How much notice
Your contract sets it, often one month. If you're paid monthly, the Employment Act's minimum is 28 days' notice in writing. Either side can end the contract without notice by paying the salary for the notice period; tick the option and the letter offers it.
Final dues and your Certificate of Service
Your employer must give you a Certificate of Service when you leave, unless you worked there for less than four weeks. Final dues usually include your salary to your last day and pay for leave you haven't taken; many companies pay them after clearance.
Resigning for personal reasons
Choose “Personal / family reasons” and the letter says so in one neutral sentence, without details. You don't owe anyone more than that.
